ITS A NEW SEASON AND A NEW START!

PRACTICE WILL BEGIN AUGUST 2ND.  PRACTICE IS HELD @ VERNON PARK MONDAY TO FRIDAY FROM 5:30 TO 7:30. 

ALL CHILDREN MUST HAVE THEIR PHYSICAL OR DR EQUIVALENT FILLED OUT WITH THE PHYSICIAN'S SIGNATURE.  IF THIS IS NOT PROPERLY FILLED OUT OR NOT HANDED IN, YOUR CHILD WILL NOT BE ABLE TO PARTICIPATE IN PRACTICE.  THIS IS A NATIONAL RULE THAT MUST BE FOLLOWED.

ALSO, ALL PAPERWORK MUST BE HANDED IN.  THE FOLLOWING PAPERWORK MUST BE GIVEN TO US BEFORE CERTIFICATIONS.

1) Registration Forms (must be signed by parent or guardian)

2) Physical form or Dr equivalent must be filled and signed my Dr.

3) If first time participant original birth certificate must be given (this is a national rule)  If child has participated in past a copy will suffice.

4) Report Card from 2009-2010 school year.  Must have all 4 quarters filled out.

5) Registration fee must be paid.
